Fill out a Ring Protection Plan Form and mail to Jostens.

- A replacement charge (plus tax and handling) is required for the replacement of your lost or stolen ring under the plan. These rates vary according to metal quality and can be found on the Ring Protection Plan Claim Form.
- The protection plan pays the remaining costs to produce an exact replica of your lost or stolen ring. If for any reason tooling is no longer available for your ring, Jostens will replace your ring with another of comparable value. The protection plan excludes replacement of diamonds and/or genuine stones.
- This protection plan is in effect for 4 years from your ring ship date, covers only your original ring for the original owner, and has a one-time use policy. If you chose to purchase the Extended Ring Protection plan, the length of the protection extends for 10 years instead of 4 – all other terms apply. When the Extended Ring Protection Plan is purchased, a consumer may submit one claim under the Standard Ring Protection Plan (if the ring was lost or stolen within four years after the ship date) and one additional claim under the Extended Ring Protection Plan (for rings lost or stolen within ten years of the ship date).
